In 2005, HOPE Family Health founders envisioned a health-care facility that would accept and care for ALL, regardless of their circumstances or background; a facility where care-givers were listeners and partners, working together with patients to build healthier lives and families. For this reason we have always offered affordable health-care not only to those who are insured, but with our sliding fee scale, to those without insurance also.
Through our new partnership with HRSA, HOPE’s Community Health Center status is allowing us to care for more patients than we ever have before. We are adding new providers, new services, dramatically improving our facility, and through these new resources we are giving HOPE to patients of all backgrounds in and throughout this area. Open five days a week, HOPE offers primary medical health-care, laboratory services, and assistance with medication. Services provided include, but are not limited to, sick exams, well-child exams, adult annual physicals, chronic disease management, care of injuries, preventive medicine, simple office procedures, and mental health services such as counseling and others.
Consistent with the goal of treating the whole person, providers address spiritual and emotional needs as well as physical needs when developing a plan of care. A spiritual support group for HOPE patients and staff is offered in the HOPE Counceling and Education Center. All are welcome to attend. HOPE Family Health is located on Hwy 52 1.1 miles east of the town of Westmoreland. We are located in the western part of Macon County, directly adjacent to Sumner County. We serve patients from Macon, Sumner, and Trousdale Counties in Tennessee and Allen County in Kentucky. Our patients are mothers, fathers, grandparents, children, teenagers, infants, and adults.
