Our Services
HOPE is a nonprofit Federally Qualified Health Center (FQHC), located in middle Tennessee and serves surrounding areas. HOPE is unique in that we provide a team-based integrated approach based on each patient's individual and unique care needs. What does that mean? Our patient's can access a broad spectrum of services under one roof, and providers from the various services coordinate care and services as a team to ensure the highest quality of care and the patient experience.

HOPE serves both insured and uninsured patients with comprehensive and affordable medical services, behavioral health services, out-patient psychiatric care, on-site pharmacy, insurance enrollment assistance, and dental health services for the whole family. Many of our services can also be done through telehealth if desired.

We welcome patients from all walks of life and seek to ensure care is not just affordable but accessible to all. As a result, HOPE offers language interpretation on-site for Spanish, American Sign Language, and many other languages.
MEDICAL SERVICES
HOPE Family Health primary care providers apply a team approach to medical care. Our team of providers is made up of family practitioners including doctors, nurse practitioners and physician’s assistants – medical experts who see you on a regular basis for wellness check-ups and get to know you over a period of several years or even decades.
IMAGING SERVICES
HOPE Family Health is proud to announce our newest health service, imaging and mammography. Our team of radiology specialists are able to provide imaging technology to diagnose and treat disease on-site. This service is available at our Westmoreland Health Center.
BEHAVIORAL HEALTH SERVICES
Our goal is to provide individuals and families in our community with high-quality, comprehensive behavioral health services so they may experience optimal emotional health, heal from painful experiences, enjoy a sense of personal wholeness and achieve their dreams.

DENTAL SERVICES - referral consultation
HOPE Family Health is happy to announce that we're able to offer many dental services at a greatly reduced rate to our HOPE patients who qualify! These discounted services include: fillings, extractions, cleanings, dental evaluations, x-rays, sealants, and topical fluoride.
ENABLING SERVICES
HOPE Family Health offers integrated services that enable our patients to easily connect to the services they need and receive help when needed. HOPE offers free Insurance Application Assistance, Prescription Assistance Navigation, Language Interpretation, a direct connect on our website to the Patient Portal and much more.
HOPE Family Health primary care providers apply a team approach to medical care. Our team of providers is made up of Family Practitioners, including Doctors, Nurse Practitioners and Physician's Assistants - medical experts who see you on a regular basis for wellness check-ups and get to know you over a period of several years or even decades. Our providers treat minor medical problems like colds and the flu and help patients with chronic, but manageable conditions, such as diabetes and high blood pressure. We are often the first to notice signs of a more serious illness or condition because we provide continuity of care by seeing patients on a regular basis for many years and noting changes in the patient's health status from one year to the next.
