Mid-Level Medical Provider

JOB SUMMARY: The certified mid-level medical provider will function as a member of a team of health professionals providing medical services to HOPE Family Health patients.  The mid-level medical provider shall, under appropriate direction and supervision by a collaborating physician, provide care, assessment, interpretation of data, and develop and implement therapeutic care plans.  The mid-level medical provider may be permitted to perform the following functions.  This is not intended to be all-inclusive or exclusive and shall be directly established by the provider’s privileges as established by HOPE Family Health in the providers contract and credentialing agreement.

Licensed Practical Nurse

JOB SUMMARY:  We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team at our Federally Qualified Health Center (FQHC). As an LPN, you will play a vital role in providing comprehensive healthcare services to our diverse patient population. Your expertise and commitment will contribute to our mission of delivering high-quality, accessible, and patient-centered care to individuals in need. Behavioral Health Therapist

JOB SUMMARY:  A full-time licensed behavioral health provider to include LMFT, LCSW, LPC/MHSP and supervised LMSW who provides behavioral health counseling to HOPE patients.  This position will assess clients and provide them treatment utilizing a variety of evidence-based counseling or therapy techniques to help guide and support optimal health functioning in each client’s life.  The program serves predominantly clients who exhibit symptoms of behavioral health disorders such as Depression, Anxiety, PTSD, Bipolar Disorder, and general mental health concerns, as well as persons who exhibit symptoms of substance use disorders.

Patient Navigator - Referrals

JOB SUMMARY:  The Patient Navigator - Referrals is responsible for managing patient referrals efficiently while ensuring accurate medical record documentation. This role provides high-quality customer service to referring providers and HOPE’s patients, coordinates specialty care, secures pre-authorizations, and supports administrative functions related to both referrals and medical records. The coordinator ensures that medical information is complete, processed correctly, and securely maintained within the electronic health record (EHR) system. This position requires independent judgment, attention to detail, and a commitment to patient confidentiality in an FQHC setting.
