Primary Care

HOPE Family Health primary care providers apply a team approach to medical care. Our team of providers are made up of Family Practitioners, Doctors, Nurse Practitioners and Physician’s Assistants – medical experts who see you on a regular basis for wellness check-ups and get to know you over a period of several years or even decades. Our providers treat minor medical problems like colds and the flu and also help patients with chronic, but manageable conditions, such as diabetes and high blood pressure. We are often the first to notice signs of a more serious illness or condition because we provide continuity of care by seeing patients on a regular basis for many years and noting changes in the patient’s health status from one year to the next.

Pediatrics

Primary health-care is essential in the early stages of life. Our medical team provides medical services to children from the time of delivery until they reach their eighteenth birthday. Our Certified family practitioners, with the assistance of our well-qualified medical support staff, work together to provide children with a personal health-care plan. Some of our pediatric services include well-child exams, immunizations, school physicals and sick visits along with a long list of other infant, child and adolescent primary care services.

Adult Medicine

Our adult medicine team provides primary care for patients who are 18 years and older. Our providers include Family Practitioners, Doctors, Nurse Practitioners and Physician’s Assistants who work together to provide patients with a personal health-care plan. Some of our adult medical services include routine physicals, annual exams, health maintenance, sick visits and specialized chronic disease care just to name a few.

Senior Medicine
Our team of medical providers understand that as the body ages there are specific things to look for, listen to, monitor, and lend caution to. We understand that seniors need special care that is tailored to their specific needs. That is why our team of Family Practitioners, Doctors, Nurse Practitioners and Physician’s Assistants work together to create health-care plans just for our senior patients. We accept most all major Medicare insurance plans and help senior patients find affordable access to their prescription drugs.
