HOPE Family Health takes pride in providing comprehensive care for our patients including primary, specialty, dental (coming soon), preventative, ancillary and mental health services. Through our various services we provide a one-stop shop approach to health-care. We offer our patients a comprehensive medical home. Our primary care services include pediatric care, family and internal medicine, well-child exams, sick exams, adult physicals, chronic disease management, care of injuries, preventative medicine, other simple office procedures and general dentistry (coming soon). Our specialty care services include geriatrics, obstetrics and gynecology, nutrition, and mental health. Our ancillary services include pharmacy-assistance and diagnostic laboratory testing and screening.

We have a very strong dedication to our patients and the standard of health-care we provide and promote. This dedication is directly tied to our goals for reducing socioeconomic, racial and ethnic barriers to health-care in the Middle Tennessee community. We provide high quality health care services for all age groups. Our highly trained physicians and clinical staff provide caring, convenient services tailored to meet the specific needs of individuals and families. Our patients are mothers, fathers, grandparents, children, teenagers, infants, and adults. Insurance Navigation

If you do not have health insurance, you may qualify for discounted services through our Sliding Fee Program. Qualification is based on family size and family income. Call us at 615-644-2000 for more information or see a staff member at any one of our health center locations. We also offer assistance with identifying potential insurance coverage through the Affordable Care Act and private insurance plans that are available on the Insurance Marketplace. Enrollment assistance is also available free of charge, even if you do not find a plan that fits your needs. Appointment slots for insurance assistance are limited so call today to schedule your free appointment.

Pharmacy Services

HOPE Family Health is excited to announce that we will be opening a fully operational on-site pharmacy that will be available to HOPE patients in the very near future. In addition to HOPE’s soon-to-come on-site pharmacy, we also offer a Prescription Assistance Program that assists uninsured patients with obtaining prescription drugs.
