HRSA Health Center Program

HOPE Family Health has been recognized for Advancing HIT (health information technology) for Quality by the Health Resources and Services Administration (HRSA). This recognition as making “notable quality improvement achievements” with one of HRSA’s new Community Health Quality Recognition (CHQR) badges is an honor for any FQHC. According to HRSA, these badges recognize achievements in access, quality, health equity, and health information technology (HIT) for the 2020 Uniform Data System (UDS) reporting period. We are honored to receive such a prestigious award.

HOPE Family Health is a recognized PCMH once again in 2021. The patient-centered medical home (PCMH) is a model of care in which patients are engaged in a direct relationship with a chosen provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the comprehensive integrated care provided to the patient, and advocates and arranges appropriate care with other qualified providers and community resources as needed.

PCMH practices develop transdisciplinary care teams to improve care coordination and care management of patient populations aiming to improve safety, efficiency, and quality in patient care.

Primary Medical Healthcare

HOPE Family Health primary care providers apply a team approach to medical care. Our team of providers are made up of Family Practitioners, Doctors, Nurse Practitioners and Physician’s Assistants – medical experts who see you on a regular basis for wellness check-ups and get to know you over a period of several years or even decades. Our providers treat minor medical problems like colds and the flu and also help patients with chronic, but manageable conditions, such as diabetes and high blood pressure. We are often the first to notice signs of a more serious illness or condition because we provide continuity of care by seeing patients on a regular basis for many years and noting changes in the patient’s health status from one year to the next.
