healing begins with HOPE
HOPE Family Health was founded in 2005 with the mission of providing affordable medical and mental health-care to all regardless of insurance status and ability to pay. HOPE is the only faith-based non for profit health center of its kind in this area. HOPE is a Federally Qualified Health Center treating patients throughout Middle Tennessee (Macon, Sumner and Trousdale counties) as well as southern parts of Kentucky.
HOPE Family Health has brought HOPE to thousands of patients. So that you can hear a couple of their stories and to learn a little more about who we are…we have created a few videos just for you…
Our Story A 2016
Our Journey A 2013
OUR GROWTH as an FQHC!
In 2012, HOPE Family Health was designated by the United States Department of Health & Human Services as a Federally Qualified Health Center, or FQHC. This status has allowed us to grow and broaden our services dramatically in the last few years. Between medical, dental, and behavioral health, we now offer more than 1200 patient encounters per month! To maintain our FQHC status and related federal funding, and because we want the very best for our patients, we operate under strict clinical, ethical, and management standards. We are proud to say that HOPE recently achieved Patient Centered Medical Home Level 2 Certification from the National Committee of Quality Assurance. We hope that if you need a provider and are not yet a patient, you will soon choose HOPE as your medical home!
OUR Additional Designations, Funding, Memberships, and Partners:
In addition to federal funding and modest clinical revenue, HOPE receives limited but important state funding as a designated Tennessee Safety Net Provider.
We are also incredibly grateful for the generous support of individual donors, contributions from local congregations, and the following local foundations: The Healing Trust, The Memorial Foundation, and the Smile 180 Foundation.
HOPE is also a proud member of the following groups and associations, who have provided invaluable support, guidance, and encouragement through the years. Thank you!
• The Christian Community Health Fellowship
• The National Association of Community Health Centers
• The Tennessee Primary Care Association
• The Rural Health Network, representing Macon, Trousdale & Sumner Counties
• The Tennessee Regional Extension Center for Health Information Technology
• The Center for Nonprofit Management
HOPE Family Health seeks to improve access to medical and mental healthcare in rural Middle Tennessee, with an emphasis on vulnerable populations such as the uninsured, underinsured, poor, homeless, children, migrant workers, and those addicted to substances. We seek to restore dignity, faith, HOPE, and health in those we serve by making them partners in the healing process and providing them with loving, compassionate care.
In 2005 HOPE Family Health founders envisioned a health center that would never turn a patient away based on insurance status or ability to pay. HOPE is grateful to be able to offer the following programs and supports to help our patients access affordable care:
Sliding Scale Fees for uninsured patients with household Income below 200% of the federal poverty level.
Free Insurance Application Assistance for patients and community members who would like to get the facts about the ACA or get help applying for insurance through the Health Insurance Marketplace, as well as Medicaid and CoverKids.
Prescription Assistance Navigation to help eligible patients apply for free or reduced-cost brand name prescriptions directly from drug manufacturers and other prescription assistance programs.
The HOPE Assistance Fund and HOPE Bridge Programs: Help with critical one-time or temporary needs for patients facing acute medical and financial hardship. Please note: Support to patients through these programs is dependent on the availability of funds. The HAF and Bridge Programs are funded exclusively by donations from congregations and generous individuals just like you!

Payment Plans for One-time Urgent Needs: In the event that a patient has an urgent medical need, as verified by a HOPE provider, and has no ability to pay on the date of service, a one-time payment plan can be arranged.
